Sam and Dean Winchester helps out Sam's imaginary friend in the eighth episode of Supernatural Season 11 entitled "Just My Imagination". Instead of airing the episode this week, it will be shown on December 2 on The CW.
In the synopsis posted on Spoiler Guide, Sam (Jared Padalecki) is shocked when his childhood imaginary friend Sully (guest star Nate Torrence) makes an unexpected appearance. Sam can't understand why he'd see Sully now but what's even more surprising is that Dean (Jensen Ackles) can see him, too. Flashbacks reveal young Sam's friendship with Sully and why he needed him.
In the video trailer of the said episode, Sully can be heard saying "It's a bloodbath" after seeing a mermaid dead in the pool. A little girl is also seen entering her room where her imaginary friend with a unicorn horn died.
The episode will also have some flashback scenes that will reveal Sam's friendship with Sully and why his imaginary friend needs him now, according to International Business Times. This episode will also temporarily take Sam and Dean's mind off of their main concern which is fixing the damage they have done after releasing the Darkness in the finale of "Supernatural" Season 10.
The reason for Sam to have an imaginary friend in his childhood days is that perhaps Sam was often left alone with no friends since his and Dean's dad (Jeffrey Dean Morgan) was always hunting down ghouls and goblins in accordance with their family's tradition, according to MNR Daily.
In addition, the emergence of Sully and the reason why Dean sees him may also have something to do with strange occurrences in where Sam and Dean are currently at. Several strange events have happened after the Darkness was set free and the killing of imaginary friends might be the consequence.
